ACP: return additional model metadata

Include metadata about usage (1x, 0.33x) and enablement in the model info

Proposed solution

The Modelnfo object allows a "_meta" field to be attached. I'd love if copilot --acp to use this field to provide additional metadata, such as enablement and usage rate, as seen in the Copilot CLI itself:

Perhaps they could be called copilotUsage and copilotEnablement? This way clients who desire will be able to provide an improved experience for the Copilot CLI.
